BC/BE Family Medicine Physician sought to join the medical staff of the Cabin Creek Health Center, a rural community health center system 20 minutes from Charleston WV, the state capital. Join colleagues dedicated to delivering quality health care in rural small town areas close to metropolitan area. The program receives federal and state support and provides care to all community members regardless of ability to pay. The center has integrated clinical pharmacy and behavioral health services, and has a clinical teaching affiliation for health professions students and residents.
The base salary is from $120,000 to $125,000 depending on experience and board status. Productivity-based salary adjustments available. Benefits include family medical benefits, professional disability coverage, life insurance, vacation and CME leave and allowance, license and membership fees. Malpractice coverage is provided though the Federal Tort Claims Act (FTCA). The coverage is unlimited and is occurrence-based (no �tail� coverage needed). In addition, the health center is an eligible site for loan state loan repayment programs.

Description of the Services and Programs of the Cabin Creek Health Systems:
- Four Community health centers in Kanawha County at Dawes, (Cabin Creek area), at Clendenin, Sissonville and Belle. Services began in 1972 at the main site.
- School-based health centers at the Riverside High School (with 1,200 students) Belle, WV and at East Bank Middle School (350 students).
- An affiliation with the WV Health Sciences Schools to coordinate and provide rural clinical rotations for health professions students including medical, nursing, physician assistant and pharmacy students. CCHS is also the lead agency for the regional AHEC program.
- Behavioral health care integrated with primary health care services at all sites.
- A pharmacy in the main health center and clinical pharmacist services coordinated with primary care services.
- Chronic disease care management including assisting patients in obtaining free and discounted medicines.
- Electronic medical record implemented in 2008.
- The current medical staff includes 3 family physicians, an internist, a med-peds physician, 3 family nurse practitioners, 3 physician assistants, 2 pharmacists, 2 clinical social workers and a part time psychologist and visiting psychiatrist.
This health center is in the mountains of southern West Virginia, within easy reach of many outdoor adventure opportunities including hiking, rock climbing, camping, white water rafting and boating, skiing, fishing and hunting. This is an opportunity for a master in social work or psychology to participate in model program of integrated behavioral health and primary care. Selected candidates will receive appropriate continuing education in the principles and methods of integrated care. In addition, the behavioral health provider will be a part of small team of behavioral health providers learning collaborative care from each other.
Integrated or collaborative care is characterized, in summary, in the following way: the behavioral health consultant (BHC) a broad range of behavioral health and behavior change challenges are addressed - from helping patients decide on lifestyle changes to coordinating care for patients with psychoses; BHCs work in the same clinical area as the primary care providers and are in regular communication with the PCPs; BHCs accept real-time or "hot hand-offs" from the PCPs; BHC encounters may be brief and there may or may not be follow-up with the BHC - the PCP usually remains the principle provider.
