(The School-Based Health Center is open Wednesdays and Fridays when school is in session.)

Appointments available: 8:30 am to 4:00 pm
(Although appointments are preferred, students may be seen on a walk-in basis, depending on the problem and availability of the staff. If necessary, appointments are available before or after school. As a service for our primary care patients we offer after hours phone access to an on-call provider on evenings, weekends, and vacations. Who We Are
Pioneer Health Center is a Primary Care Health Center conveniently located East Bank Middle School. We provide healthcare to school age children and their families, school staff and the community. We are here to provide care to students of busy working families, students with/without transportation and students with or without insurance. We provide care to students efficiently so they miss very little classroom time and parents can have peace of mind knowing that we will be here to provide all of their primary care when they need us.

We are part of the Cabin Creek Health Systems, your community owned non-profit health center.

Pioneer Health Center Providers

Mary Grandon, PA-cMary Grandon, PA-c
Mary Grandon is a certified Physician Assistant who has been providing care at the Riverside Health Center since 2006. Mary graduated with a Bachelor of Science in Medical Science from Alderson Broaddus in 1992. Before coming to work at Riverside, Mary worked with school based health centers in Clay County.

Mary currently resides in Clay County with her husband Eric. In her spare time she enjoys mission work in Haiti.

 

Sam Farley, MSW Sam Farley, MSW
Sam Farley is a behavioral health consultant providing care at the Indian Health Center. Sam received his Bachelor of Arts in Social Sciences from Antioch University in 1978 and his Master’s of Social Work from West Virginia University in 1989. Sam spent the last seventeen years working at various community health agencies as a team leader for children’s health services.

Sam lives in St Albans with his wife 30 years; he has one son. In his spare time Sam enjoys listening to Jazz and Blues music.
