Patient Assistance Programs

CCHS offers to help patients apply for programs to aid with the cost of health care. Based on household size and income, you and your family may qualify for one of the programs.

Sliding Fee/CAP
The Sliding Fee/CAP program is designed for adults who do not qualify for other health care programs. Through these programs people can qualify for low cost health care at any of the sites within CCHS. They may also use the CAP card at CAMC. To determine if you qualify for the program, click here to download and print an application for the sliding scale services, complete the application, and bring the completed application and your household’s proof of income to the closest CCHS health center.

WV Children’s Health Insurance Program (CHIP)

WVCHIP is designed to provide health care for children and young adult’s ages 0 to 19 years old. Services include: medical, dental, vision, hospitalization, ER services, prescription services, mental health services and more.

WV Medicaid

For Pregnant Women and Adults

Pregnant Women Medicaid
To apply, you must bring in the household’s monthly income and proof of pregnancy statement. The application can be done at CCHS.

Adult Medicaid
The actual application must be completed at WV Division of Health and Human Resources but the patient assistance office at CCHS can help you set up an apportionment, and let you know what to take with you.

Medicare

Medicare is a health insurance program for people age 65 or older, people under the age of 65 with certain disabilities, and people of all ages with End-Stage Renal Disease (permanent kidney failure requiring dialysis or a kidney transplant.

Medicare Savings Program
The Medicare Savings Program pays for some of the costs of Medicare, including the Part B premium, deductibles, and co pays. To determine if you qualify for this program bring in your household’s monthly income, copies of all utility bills, house and car payments, checking and savings account statements, and any other proof of assets or debts to the patient assistance office.

WV Breast and Cervical Cancer Screening Program (BCCSP)

To qualify for BCCSP you must be female, between the ages of 25 – 64, have an income at or below 200% of the current Federal Poverty Level, and uninsured. Patients might also qualify if they are underinsured and their medical insurance does not cover BCCSP screening services.
